Requesting a Portal Account is a two-step process. The first step is to provide your Certification # , Last Name, and email address. Once submitted, click the link in the verification email to complete the request.

Access to features that require extra permissions will be restricted until your account has been reviewed. See your supervisor or system administrator for more information.

Your Certification # and Last Name are required to proceed.

Portal User Name / Email Address

A valid email address is required for a portal account. The email address must be unique (no other portal user account can use the same email address). Upon successful registration, a verification email will be sent. Your registration will not be complete until you respond to the verification email.
